PPO, Medicare Part C, & Accident Reimbursement

We are A CMS Qualified Provider

We’re a qualified out-of-network provider muscular and soft tissue pain (chronic or recurring). While we don’t accept copays, you can apply for reimbursement for any of our services.

This only works for most PPO policies, Medicare (Part C), and Auto Accident or Workman’s Compensation situations.

If approved, they will pay for part or all of your expenses, depending upon your coverage.

This will not apply for HMO, Medicaid, or standard Medicare policies.

Just follow the process below to get the best possible outcome.

How To Get Approved

Step 1 — Contact Your Doctor

Unless you know you have coverage for CAM (Chiropractic, Acupuncture, and Massage), you will need a referral from your primary care doctor, physical therapist or chiropractor. Just explain why you need our therapy and if approved, they need to send us a referral. You can email them our details for their records [tap here] including our NPI# and FAX.

Step 2 — Send Us The Referral

If they do not fax direct to us, and you receive the referral document, please email the basic information within the referral [tap here] Include your name, diagnostic code, and diagnostic description. We will match it with an appropriate CPT treatment code and provide you that info by return email.

Step 3 — Contact Your Carrier Billing Dept

You should then contact your insurance billing to confirm what coverage you will receive by providing the Diagnostic Code and the Treatment code to confirm your coverage.

Step 4 — Apply For a SuperBill

Once you have their referral, our CPT code, and approval from billing, you can apply for a Superbill with us. Just complete the form below, including all of your insurance group number details etc.

At the end of each month we will send out a consolidated Superbill of all of the therapies that you have prepaid.

Treatment CodeS

For informational purposes, here is a guide of the CPT codes we can offer:

CPT 97124 – Thai Sport Massage.  Massage therapy techniques to increase circulation, promote tissue relaxation to the muscles, relieve minor pain, and maintain musculoskeletal (MSK) wellness.

CPT 97140 – Neuromuscular Therapy. Advanced therapy for acute and chronic pain. Provides soft tissue manipulation, joint mobilization, trigger point therapy, and myofascial release.

CPT 97140 – Prehab Therapy.  Advanced therapy for muscular tension, movement dysfunction, and posture correction that contributes to pain and poor performance. Uses soft tissue manipulation, joint mobilization, trigger point therapy, myofascial release, and PNF techniques. Treatment is targeted by an MSK diagnosis using Phy Body Analysis.

CPT 97112 – Deepwave Therapy.  Advanced techniques for chronic pain, injury and surgery recovery. Uses neuromuscular re-education of movement, balance, coordination, kinesthetic sense, and proprioception. Rapidly resolves pain, increases strength, and corrects compensation using patented technology from ARPwave.
